New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

We will ask for 2 forms of identification one of which should preferably be photo I.D. such as a passport or driving license and one should have your current address i.e. utility bill, bank statement or rent agreement.

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. If you take repeat medication please ensure you have an adequate supply before registering with us as we may not be able to issue any medication straight away.

These forms can be collected from Reception

All new adult patients are required to have a New Patient Health Check with a member of the Nursing team  to ensure that information and any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. It is preferable for this to be done prior to a Doctors appointment.
